Do you know what kind of things I need to submit? I assume, name/address, receipt, and UPC?

ReplyDeleteI found out you need a 3x5 card with name and address, original cash register receipt, and UPC. The UPC can be written on the 3x5 card since it is a sticker on the Tums package and hard to remove.
