You'll find a TMF form for Tums QuikPak 24 ct or larger (up to $6.99). You'll need to submit the form and UPC from your purchase dated between 5/18/08 & 6/30/08. Form must be received by 7/15/08. In the same flyer there is a $1 off coupon for this product (which they may deduct from your rebate).
These Tums QuikPaks are the little packs of "beady" medicine that melts on your tongue. I have noticed a lot of companies starting to make these; I've seen them mainly in kids products so far (besides these Tums!).
You can print a coupon for $2.25 off from the Tums website as well.
